This issue has been posted a few times by different users, but nobody has posted the reason that it happens... So I'll do that now - and everyone can refer back to this post in the future!!!

It'll often happen if you answer the question too close to the 5:00 time limit. Most of the time the in-game flash countdown runs slower than real time (by how much depends on the speed of your computer, and how many programs / browser windows you have running), but the page won't re-initialize on your computer until your FLASH timer runs out, even if the ACTUAL 5:00 has elapsed (as per the Doomlord server, which is not flash-based, and therefore is 99.999999999999999% accurate).

What I've learned is if it's in the last 30 seconds of the countdown, refresh the page (usually F5) to get the Flash timer to reset to the Doomlord timer - because if you answer too late, then the question has already rolled over to the next one, and you have a 75% chance of getting that one wrong AS WELL. The time lost in refreshing the page may cost you a chance at the current question, but its better than getting two wrong.

This is one of the reasons why this can happen, another is that you accidentally click twice on the submit button, or you hit it again, thinking that you did not hit it the 1st time, to avoid this refresh as Vidriel said.
And supposedly there is also a bug in Internet Explorer that causes a double click to be sent. (I don't use IE, so I can't confirm this one)

Green

Pure water has a slight blue colour, but small quantities of water does seem clear to the human eye.

What is the dot about the letter i called?

(or something like that)

Answers are Tittle (marked as correct) and there is also a choice of "Dot". That is ambiguous as a Tittle is defined as the dot above the letters such as "i" and "j". If tittle = dot, then there are really two correct answers, even the questions calls it a dot...

There is a question similar to this already but I also think it's ambiguous, the answers are all fairly similar and slaves were sent to work in mines, coffee, sugar and cotton plantations. Does it require specific knowledge on the slave trade? I had no idea where the majority were sent and it wasn't easy for me to find either...

Reminds me of the stupid Lobster Blood question. Everywhere I read that it's clear with a hint of blue, yet guess what the correct answer is - blue! Yet here we have clear with a hint of blue, and the answer is ------- Clear!!!

This is wrong. Pisces was the previous age. The next age is Capricorn.

The "age" order is the exact zodiac, but in reverse order.

There's a Monopoly question about which city the board game made "famous" and the correct answer is marked as London - I've never even played that version here in the USA - Atlantic City is the version that is famous here and according to Wikipedia...

Waddington's version, their first board game, with locations from London substituted for the original Atlantic City ones, was first produced in 1936

I am also certain London was "famous" long before Monopoly...
